I hate this. I am so freaking tired of "cat hasn't eaten in days" presentations. I have had a run of them and they are ALL BAD.
Neutered male cat hasn't eaten in 2 days = urethral blockage, owner has no money, euthanasia.
Intact male cat hasn't eaten in 2 days = saddle thrombus, died overnight.
Spayed female cat hasn't eaten in 3 days = probable stomach cancer, progressed to vomiting blood, haven't heard from owner today but I expect she died over the weekend.
Spayed female cat hasn't eaten in 3-4 days = no freaking idea, blood work is okay, owners don't want x-rays or ultrasound, started her on antibiotics and antacids and hoping.
Intact male cat hasn't eaten in 5 days = severe anemia of unknown cause, owners can't afford referral or blood transfusion, so started on antibiotics and steroids and hoping.
